Indications
- Prophylactic treatment of urinary tract infections
- Suppressive treatment of urinary tract infections
- Long-term therapy for recurrent urinary tract infections
- Use after eradication of infection by other antimicrobial agents
- Treatment of infections caused by susceptible bacteria
- Consideration of culture and susceptibility information in therapy selection
Pharmacology
- Methenamine component hydrolyzes to formaldehyde in acid urine
- Hippuric acid keeps urine acid
- Active against E. coli, enterococci, and staphylococci
- Resistant to Enterobacter aerogenes
- Requirement of sufficiently acid urine to inhibit urea-splitting organisms
Dosage & Administration
- 1 tablet twice daily for adults and pediatric patients over 12 years
- 1/2 to 1 tablet twice daily for pediatric patients 6 to 12 years
- Greater antibacterial activity in acid urine
- Monitoring efficacy through repeated urine cultures
Contraindications
- Contraindicated in patients with renal insufficiency
- Contraindicated in patients with severe hepatic insufficiency
- Contraindicated in patients with severe dehydration
- Not recommended for patients taking sulfonamides
Pregnancy & Lactation
- Safe use not established in early pregnancy
- Safety suggested but not definitely proved in the last trimester
- No adverse effects on fetus in pregnant rats and rabbits
- Interference with laboratory tests of urine estriol
- Enzymatic hydrolysis recommended to circumvent interference
Precautions & Warnings
- Prescription only for proven or strongly suspected bacterial infections
- Maintenance of acid urine pH in treating infections due to urea-splitting organisms
- Recommendation for periodic liver function studies during treatment
Use in Special Populations
- Clinical studies did not include sufficient subjects aged 65 and over
- No identified differences in responses between elderly and younger patients
- Cautious dose selection for elderly patients
- Contraindicated in patients with renal or severe hepatic insufficiency
