Indications

  • Treatment of type 2 diabetes mellitus
  • Used as monotherapy or in combination with other oral antidiabetic agents or with insulin
  • Reduction of diabetic complications in overweight type 2 diabetic adult patients

Pharmacology

  • Biguanide type oral antihyperglycemic drug
  • Lowers both basal and postprandial plasma glucose
  • Improves insulin sensitivity
  • Decreases hepatic glucose production
  • Decreases intestinal absorption of glucose

Dosage & Administration

  • Dosage individualized based on effectiveness and tolerance
  • Usual starting dose for adults: 500mg-850mg per day
  • Usual starting dose for children: 500mg per day
  • Dosage increases should be made in specific increments
  • Do not exceed maximum recommended daily doses

Interaction

  • Potential interaction with Furosemide and Nifedipine
  • Effect on cationic drugs eliminated by renal tubular secretion
  • No effect on Cimetidine pharmacokinetics
  • Drugs that tend to produce hyperglycemia may interact with Informet XR

Contraindications

  • Hypersensitivity to active substance or excipients
  • Acute metabolic acidosis or severe renal failure
  • Conditions that may alter renal function or cause tissue hypoxia

Side Effects

  • Hemolytic anemia
  • Lactic acidosis
  • Decrease of vitamin B12 absorption
  • Taste disturbance
  • Gastrointestinal disorders
  • Liver function test abnormalities
  • Skin reactions

Pregnancy & Lactation

  • Increased risk of congenital abnormalities and perinatal mortality with uncontrolled diabetes during pregnancy
  • Metformin excreted into human breast milk
  • Breastfeeding not recommended during metformin treatment

Precautions & Warnings

  • Substantial excretion by the kidney
  • Risk of lactic acidosis increases with impaired renal function
  • Lower vitamin B12 level
  • Increased risk of hypoglycemia when used in combination with insulin
  • Regular monitoring of renal function advised

Use in Special Populations

  • Adjustment of metformin dosage based on renal function in elderly subjects
  • Confirmation of type 2 diabetes mellitus before treatment initiation in children
  • Regular assessment of renal function necessary in all diabetics

Overdose Effects

  • Hypoglycemia has not been seen with Informet XR doses up to 85 gm
  • Lactic acidosis is a medical emergency and must be treated in hospital
  • Hemodialysis is the most effective method to remove lactate and Informet XR
