Indications

  • Treatment of type 2 diabetes mellitus
  • Monotherapy for adults and combination therapy for both adults and children
  • Reduction of diabetic complications in overweight patients
  • First-line therapy after diet failure

Pharmacology

  • Biguanide type oral antihyperglycemic drug
  • Lowers basal and postprandial plasma glucose levels
  • Different mechanism of action compared to sulfonylureas
  • Does not produce hypoglycemia
  • Decreases hepatic glucose production
  • Improves insulin sensitivity

Dosage & Administration

  • Available as immediate release and extended release tablets
  • Individualized dosage based on effectiveness and tolerance
  • Maximum recommended daily doses for adults and children
  • Special dosage considerations for renal impaired patients

Interaction

  • Minimal effects on Nifedipine
  • Potential interaction with cationic drugs eliminated by renal tubular secretion
  • Certain drugs leading to hyperglycemia
  • No effect on Cimetidine pharmacokinetics

Contraindications

  • Hypersensitivity to active substance or excipients
  • Acute metabolic acidosis
  • Severe renal failure
  • Acute conditions altering renal function
  • Conditions causing tissue hypoxia

Side Effects

  • Hemolytic anemia
  • Lactic acidosis
  • Decrease in vitamin B12 absorption
  • Peripheral neuropathy in patients with vitamin B12 deficiency
  • Taste disturbance
  • Encephalopathy
  • Gastrointestinal disorders
  • Liver function test abnormalities or hepatitis
  • Skin reactions

Pregnancy & Lactation

  • Increased risk of congenital abnormalities and perinatal mortality with uncontrolled diabetes during pregnancy
  • Breastfeeding not recommended due to limited data

Precautions & Warnings

  • Substantial excretion by the kidney
  • Lowering of vitamin B12 levels
  • Increased risk of hypoglycemia with insulin or insulin secretagogue
  • Monitoring of renal function advised
  • Discontinuation before surgery and reinstatement post-surgery
  • Not advised in dehydrated or seriously infected patients

Use in Special Populations

  • Adjusted dosage for elderly subjects based on renal function
  • No impact on growth and puberty observed during controlled clinical studies
  • Caution recommended for children aged between 10 and 12 years
  • Regular assessment of renal function necessary
  • Special caution in situations where renal function may become impaired

Overdose Effects

  • Hypoglycemia not seen at high doses
  • Lactic acidosis occurred at high doses
  • Hemodialysis for treatment of lactic acidosis
