Indications

Pharmacology
- Proton pump inhibitor
- Specific inhibition of the H+/K+-ATPase in the gastric parietal cell
- Enhanced acid control compared to racemic proton pump inhibitors
- Enteric-coated pellet formulation of esomeprazole magnesium
- Peak plasma levels (Cmax) at approximately 1.5 hours (Tmax)
- 97% bound to plasma proteins
- Extensively metabolized in the liver
- Plasma elimination half-life of approximately 1–1.5 hours
Dosage
- 20 mg or 40 mg Once Daily for 4-8 Weeks for healing of erosive esophagitis
- 20 mg Once Daily for maintenance of healing of erosive esophagitis
- 20 mg Once Daily for symptomatic GERD
- 40 mg Once Daily for Helicobacter Pylori eradication
- 20-80 mg Once Daily for Zollinger-Ellison Syndrome
- 20-40 mg Once Daily for Acid-related Dyspepsia
- 20 mg Once Daily for 2-4 weeks for Duodenal ulcer
- 20-40 mg Once Daily for 4-8 weeks for Gastric ulcer
- IV Injection: 20 mg or 40 mg once daily for GERD with Erosive Esophagitis
- IV Infusion for pediatric patients based on body weight
Administration
- Tablet or capsule should be swallowed whole and taken one hour before a meal
- Oral Suspension should be taken into a small glass containing 15 ml of water and stirred well
- IV Injection should be administered over a period of at least 3 minutes
Interaction
- Extensively metabolized in the liver by CYP2C19 and CYP3A4
- Inhibition of CYP2C19 may interfere with the clearance of diazepam
- May interfere with the absorption of drugs where gastric pH is crucial for bioavailability
Contraindications
- Known hypersensitivity to any of the formulation

Pregnancy & Lactation
- No adequate and well-controlled studies in pregnant women
- No studies on excretion of esomeprazole in milk
- Breastfeeding should be discontinued if esomeprazole use is considered essential
Precautions & Warnings
- Symptomatic response to therapy does not preclude the presence of gastric malignancy
- Capsules should be taken at least one hour before meals
- Antacids may be used while taking the product
Use in Special Populations
- Safety and effectiveness in pediatric patients not established
- No overall differences in safety and efficacy between elderly and younger individuals

- Acute toxicity in animal studies at high doses
- No specific antidote known
