Indications

Pharmacology
- Third generation semisynthetic cephalosporin antibiotic
- Orally administered bactericidal antibiotic
- Bactericidal against a broad spectrum of gram positive and gram negative bacteria
- Interferes in the synthesis of bacterial cell wall
- Highly stable in the presence of beta-lactamase enzyme
- Approximately 40% to 50% absorption with or without food
Dosage & Administration
- Usual course of treatment is 7 days
- Can be continued for up to 14 days if required
- Recommended adult dosage is 200-400 mg daily
- Elderly patients may be given the same dose as recommended for adults
- Recommended dosage for children is 8 mg/kg/day
- Specific daily doses in terms of volume of suspension for children of different age groups
- Different dosage for typhoid in children
- Dosage adjustment in severe renal impairment
- Recommended dose may not be exceeded in patients with creatinine clearance less than 20 ml/min
- Dosing regimen for patients on chronic ambulatory peritoneal dialysis or hemodialysis
Interaction
- Increased prothrombin times in some patients
- Care required in patients receiving anticoagulant therapy
Contraindications
- Known hypersensitivity to cephalosporin antibiotics
Side Effects
- Mostly mild and self limiting adverse reactions observed in clinical trials
- Frequent gastrointestinal disturbances such as diarrhea and stool changes
- Less frequent gastrointestinal side-effects including nausea, abdominal pain, dyspepsia, vomiting, and flatulence
- Reported cases of pseudomembraneous colitis
- Central nervous system effects like headache and dizziness
- Hypersensitivity reactions in the form of rash, pruritis, urticaria, drug fever, and arthralgia
- Reported cases of thrombocytopenia, leukopenia, eosinophilia, and reversible mild transient change in liver and renal function tests
- Other possible reactions include genital pruritis and vaginitis
Pregnancy & Lactation
- No adequate and well-controlled studies in pregnant women
- Use during pregnancy only if clearly needed
- Caution required when administered to nursing women
Precautions & Warnings
- Caution required for patients with hypersensitivity to other drugs
- Caution required for penicillin-sensitive patients
- Caution advised for patients with markedly impaired renal function
- Altered normal flora of the colon and risk of antibiotic-associated diarrhea
- Not recommended for use in pregnancy or nursing mothers unless considered essential
Overdose Effects
- Gastric Lavage may be indicated in case of overdose
- No specific antidote exists
- Not removed in significant quantities by hemodialysis or peritoneal dialysis
- Adverse reactions in healthy adult volunteers receiving single doses up to 2 g did not differ from the profile seen in patients treated at recommended doses
