Indications

  • Reduce rate of thrombotic cardiovascular events
  • Manage with percutaneous coronary intervention
  • Unstable angina
  • Non-ST-elevation myocardial infarction
  • ST-elevation myocardial infarction

Pharmacology

  • Inhibitor of platelet activation and aggregation
  • Irreversible binding to P2Y12 class of ADP receptors

Dosage & Administration

  • Initiate with 60 mg oral loading dose
  • Continue with 10 mg once daily
  • Consider 5 mg once daily for patients <60 kg
  • Take with or without food
  • Take aspirin daily

Interaction

  • Increases risk of bleeding when coadministered with warfarin
  • May increase risk of bleeding when coadministered with NSAIDs
  • Can be administered with certain drugs and inhibitors of cytochrome P450 enzymes

Contraindications

  • Active pathological bleeding
  • History of prior transient ischemic attack or stroke

Pregnancy & Lactation

  • No adequate and well-controlled studies in pregnant women
  • Potential risk to nursing infant
  • Use during nursing justified only if potential benefit to mother

Precautions & Warnings

  • Increased risk of bleeding in CABG
  • Premature discontinuation increases risk of stent thrombosis, MI, and death

Use in Special Populations

  • Pediatric use safety and effectiveness not established
  • Increased risk of fatal bleeding events in patients >75 years old
  • No dosage adjustment necessary for renal impairment
  • No dosage adjustment necessary for mild to moderate hepatic impairment

Overdose Effects

  • Lethality observed in rats after administration of 2000 mg/kg
  • Platelet transfusion may restore clotting ability
  • Active metabolite not likely to be removed by dialysis
