Pharmacology

  • Immediate release Esomeprazole Magnesium layer
  • Enteric-coated Naproxen core
  • Prostaglandin synthesis inhibition
  • Proton pump inhibitor action

Dosage

  • Adults dosage: 375 mg naproxen/20 mg of esomeprazole
  • Adolescent dosage: 375 mg naproxen/20 mg of esomeprazole or 500 mg naproxen/20 mg of esomeprazole

Administration

  • Should be taken at least 30 minutes before meals

Interaction

  • Reduced antihypertensive effect with NSAIDs
  • Increased bleeding risk with warfarin
  • Interference with drug absorption by Esomeprazole
  • Affects absorption with high-fat food

Contraindications

  • Hypersensitivity to tablet components
  • History of allergic-type reactions after taking aspirin or other NSAIDs
  • Use during coronary artery bypass graft surgery

Side Effects

  • Decreased incidence of gastrointestinal side effects
  • Risk of arterial thrombotic events
  • Gastrointestinal adverse events
  • Oedema, hypertension, cardiac failure
  • Most commonly observed adverse events are gastrointestinal in nature

Pregnancy & Lactation

  • Avoid during first and second trimester
  • Contraindicated during third trimester
  • Not recommended during breastfeeding
  • May impair female fertility

Precautions & Warnings

  • Avoid combination with NSAIDs and cyclooxygenase-2 selective inhibitors
  • Minimize undesirable effects by using lowest effective dose
  • Risk factors for NSAID related gastro-intestinal complications
  • Increased risk of adverse reactions in older people
  • Increased risk of GI bleeding, ulceration or perforation with NSAIDs
  • Monitoring required for patients with hypertension and/or congestive heart failure
  • Risk of renal papillary necrosis and other renal injury

Use in Special Populations

  • Caution in elderly patients
  • Not recommended for patients with moderate to severe renal impairment
  • Close monitoring in patients with mild to moderate hepatic impairment

  • Symptoms related to Naproxen overdose
  • Symptoms related to esomeprazole overdose
  • Symptomatic and supportive care required
  • No specific antidote known
