Indications
- Adjunctive treatment of all forms of parkinsonian syndrome
- Symptomatic treatment of idiopathic, postencephalitic and arteriosclerotic parkinsonian
- Control of extrapyramidal symptoms including pseudoparkinsonian, acute dystonic reactions and akathisia induced by neuroleptic drugs
Pharmacology
- Antimuscarinic antiparkinsonian agent
- Synthetic tertiary amine
- Corrects relative cholinergic excess in parkinsonian due to dopamine deficiency
- Absorbed from G.I. tract
- Acts within 5 to 20 minutes
- Duration of effect up to 4 hours
Dosage & Administration
- Administered orally
- Tablet dosage for parkinsonism: 2.5 mg 3 times a day, then 5 mg 3 times a day, occasionally 5 mg at bedtime
- Tablet dosage for drug induced extrapyramidal symptom: Initially 2.5 mg 3 times a day, increased by 2.5 mg increment per day
- IM or IV injection dosage: 5-10 mg, repeated if necessary after 20 minutes; maximum 20 mg daily
Interaction
- Anticholinergic activity may be increased by certain agents
- Reduction in ketoconazole absorption
Contraindications
- Caution in children and geriatric patients
- Cautious use in patients with diarrhoea, cardiovascular disease, glaucoma, urinary retention, hepatic or renal impairment
- Safety during pregnancy not established
- No data available on drug excretion in breast milk
Side Effects
- Dryness of the mouth at usual dosage levels
- Occasional mydriasis, blurred vision, G.I. effects, allergic reaction, muscular weakness
- High doses may cause vertigo, confusion, and hallucination
- Adverse effects can be minimized by adjustment of dosage and administration after meal
Pregnancy & Lactation
- The safe use in pregnancy, lactation, or in women of childbearing age requires weighing potential benefits against possible hazards
Precautions & Warnings
- Caution in case of hepatic & renal impairment, children, elderly, pregnancy, and lactation
- Avoid abrupt withdrawal to prevent rebound Parkinsonism symptoms
- Potential precipitation of a psychotic episode in patients with mental disorders
