Indications
- Essential hypertension
- Angina pectoris
- Vasospastic angina
Pharmacology
- Calcium-channel blocker
- Vasodilation
- Reduced peripheral resistance
- Increased coronary blood flow
- Reflex increase in coronary heart rate
Dosage & Administration
- Usual dose: 5 mg once daily
- Maximum dose: 10 mg once daily
- May be taken without regard to meals
Interaction
- Potentially hazardous interactions with p-blockers
- No interaction with Digoxin
- No interaction with Cimetidine
- No significant alteration of Warfarin effect
- Food does not alter the absorption
Contraindications
- Hypersensitivity to dihydropyridine derivatives
- Pregnancy

- Flushing

- Hypotension
- Peripheral edema
- Gastrointestinal disturbances
- Increased micturition frequency
- Lethargy
- Eye pain
- Mental depression
- Paradoxical increase in ischaemic chest pain
- Excessive fall in blood pressure
- Cerebral or myocardial ischemia
- Transient blindness
- Rashes
- Fever
- Abnormalities in liver function
Pregnancy & Lactation
- Pregnancy Category C
- Limited information on excretion in human milk
- Recommendation to discontinue nursing while administered
Precautions & Warnings
- Caution in patients with hepatic impairment
- Caution during pregnancy and breast feeding
Use in Special Populations
- Different dosage for children and elderly
- No established dosage recommendations in severe hepatic impairment
- Not dialysable
Overdose Effects
- Excessive peripheral vasodilatation
- Reflex tachycardia
- Systemic hypotension
- Shock
- Clinically significant hypotension
- Active cardiovascular support required
- Vasoconstrictor may be helpful
- Intravenous calcium gluconate may be beneficial
- Gastric lavage may be considered
- Charcoal administration shown to reduce absorption rate
- Dialysis not likely to be beneficial
