Indications
- Reduction of elevated total cholesterol

Pharmacology
- Inhibition of HMG-CoA reductase
- Reduction of LDL production

- Inhibition of cholesterol absorption by the small intestine
- Molecular target of ezetimibe - Niemann-Pick C1-Like 1 (NPC1L1)
- Reduction of hepatic cholesterol stores
- Increase in clearance of cholesterol from the blood
Dosage & Administration
- Dosage range: 10/10 mg to 80/20 mg daily
- Recommended starting dose: 10/10 mg or 20/10 mg daily
- Recommended starting dose for patients with larger reduction in LDL-C: 40/10 mg daily
- Dosage for patients with homozygous familial hypercholesterolemia: 40/10 mg or 80/10 mg daily
- Administration as a single dose at any time of the day
- Analysis of lipid levels within 2 or more weeks after initiation
Interaction
- Increased risk of myopathy with concurrent administration of fibric acid derivatives
- Risk of myopathy/rhabdomyolysis with gemfibrozil
- Enhanced risk of skeletal muscle effects when used in combination with niacin
- Monitoring recommended for patients taking digoxin
- Consideration of increased AUC values for norethindrone and ethinyl estradiol when selecting an oral contraceptive
Contraindications
- Active liver disease
- Persistent elevations of hepatic transaminase levels
- Hypersensitivity to any component

- Rhabdomyolysis
- Myopathy
- Liver enzyme abnormalities
- Myalgia
- Abdominal pain
- Increased hepatic enzymes
Pregnancy & Lactation
- Pregnancy Category X
- Contraindicated in pregnant women
- Excretion in human milk uncertain
- Discontinuation during pregnancy unlikely to impact long-term outcomes
Precautions & Warnings
- Temporary withholding or discontinuation in case of myopathy
- Liver enzyme tests recommended prior to initiating therapy
- Caution with drugs that may decrease endogenous steroid hormones
- Potentially increased risk of renal failure secondary to rhabdomyolysis
Use in Special Populations
- Safety and effectiveness not established in pediatric patients
- No dosage adjustment necessary in geriatric patients

- Symptomatic treatment and supportive measures instituted as required
