Indications
- Adjunct to diet and exercise
- Improves glycemic control
- Type 2 diabetes mellitus
Pharmacology
- Inhibits DPP-4 enzyme
- Increases active incretin hormones
- Stimulates insulin release
- Reduces glucagon levels
- Inhibitor of Sodium-glucose co-transporter 2 (SGLT2)
- Reduces renal reabsorption of glucose
- Increases urinary glucose excretion
Dosage & Administration
- Recommended dose: 10 mg Empagliflozin and 5 mg Linagliptin once daily
- Increased dose: 25 mg Empagliflozin and 5 mg Linagliptin once daily
- Assess renal function before initiating
- Do not initiate if eGFR is below 45 mL/min/1.73 m²
- Discontinue if eGFR falls below 45 ml/min/1.73 m²
Interaction
- Coadministration with diuretics
- Coadministration with Insulin or Insulin secretagogues
- Rifampin decreased Linagliptin exposure
- Increase in urinary glucose tests
- Interference with 1,5-anhydroglucitol (1,5-AG) Assay
Contraindications
- Severe renal impairment
- End-stage renal disease or dialysis
- Hypersensitivity reactions to Linagliptin
- Serious hypersensitivity reaction to Empagliflozin

- Urosepsis and Pyelonephritis
- Hypoglycemia with concomitant use with Insulin and Insulin Secretagogues

Pregnancy & Lactation
- Not recommended during second and third trimesters
