Indications
- Reduction of elevated total cholesterol
- Reduction of low density lipoprotein cholesterol
- Reduction of triglycerides
- Increase in high-density lipoprotein cholesterol
- Indicated for primary hyperlipidemia
- Indicated for homozygous familial hypercholesterolemia
- As an adjunct to diet
- Reduction of non-high-density lipoprotein cholesterol
Pharmacology
- Atorvastatin lowers plasma cholesterol and lipoprotein levels
- Ezetimibe reduces blood cholesterol by inhibiting absorption of cholesterol by the small intestine
- Ezetimibe inhibits intestinal cholesterol absorption by 54%
- Atorvastatin increases the number of hepatic LDL receptors on the cell surface
Dosage & Administration
- Dosage range is 10/10 mg to 80/20 mg daily
- Recommended starting dose is 10/10 mg or 20/10 mg daily
- Can be administered as a single dose at any time of the day, with or without food
- Dosage in patients with homozygous familial hypercholesterolemia is 40/10 mg or 80/10 mg daily
- Regular analysis of lipid levels within 2 or more weeks after initiation
Interaction
- Increased risk of myopathy during treatment with statins
- Coadministration with fibric acid derivatives increases risk of myopathy
- Coadministration with strong CYP3A4 inhibitors enhances risk of skeletal muscle effects
Contraindications
- Active liver disease
- Unexplained persistent elevations of hepatic transaminase levels
- Hypersensitivity to any component
Side Effects
- Rhabdomyolysis
- Myopathy
- Liver enzyme abnormalities
- Myalgia
- Abdominal pain
Pregnancy & Lactation
- Pregnancy Category X
- Contraindicated in women who are or may become pregnant
- Serum cholesterol and triglycerides increase during normal pregnancy
- Not known whether atorvastatin is excreted in human milk
Precautions & Warnings
- Cases of myopathy have been reported, caution with colchicine
- Recommended liver enzyme tests prior to therapy initiation
- Caution if administered concomitantly with drugs that may decrease levels or activity of endogenous steroid hormones
Use in Special Populations
- Safety and effectiveness not established in pediatric patients
- No dosage adjustment necessary in geriatric patients
Overdose Effects
- No specific treatment of overdosage can be recommended
- The patient should be treated symptomatically
