Indications
- Reduce rate of thrombotic cardiovascular events
- Manage with percutaneous coronary intervention
- Unstable angina
- Non-ST-elevation myocardial infarction
- ST-elevation myocardial infarction
Pharmacology
- Inhibitor of platelet activation and aggregation
- Irreversible binding to P2Y12 class of ADP receptors
Dosage & Administration
- Initiate with 60 mg oral loading dose
- Continue at 10 mg once daily
- Consider 5 mg once daily for patients <60 kg
- Take with or without food
- Co-administer with aspirin (75 mg to 325 mg) daily
Interaction
- Increase risk of bleeding with warfarin
- Increase risk of bleeding with NSAIDs
- Administer with inducers or inhibitors of cytochrome P450 enzymes
- Administer with aspirin, heparin, GPIIb/IIIa inhibitors, statins, and drugs that elevate gastric pH
Contraindications
- Active pathological bleeding
- History of prior transient ischemic attack or stroke

Pregnancy & Lactation
- No adequate and well-controlled studies in pregnant women
- Not known if excreted in human milk
- Use during nursing only if benefit justifies potential risk to the nursing infant
Precautions & Warnings
- Increased risk of bleeding in patients undergoing CABG
- Premature discontinuation increases risk of stent thrombosis, MI, and death
Use in Special Populations
- Safety and effectiveness in pediatric patients not established
- Increased risk of fatal bleeding events in geriatric patients > 75 years
- No dosage adjustment for renal impairment
- No dosage adjustment for mild to moderate hepatic impairment
Overdose Effects
- Lethality observed after administration of 2000 mg/kg in rats
- Platelet transfusion may restore clotting ability
