Indications
- Reduces rate of thrombotic cardiovascular events
- Indicated for acute coronary syndrome
- Manages patients with unstable angina or myocardial infarction
Pharmacology
- Inhibitor of platelet activation and aggregation
- Binds irreversibly to P2Y12 class of ADP receptors
Dosage & Administration
- Initial 60 mg oral loading dose
- Followed by 10 mg once daily
- Consider 5 mg once daily for patients <60 kg
- Should be taken with aspirin daily
Interaction
- Increases risk of bleeding when coadministered with warfarin or NSAIDs
- Can be administered with certain drugs including aspirin, heparin, and statins
Contraindications
- Active pathological bleeding
- History of prior transient ischemic attack or stroke
Side Effects
- Bleeding
- Thrombotic thrombocytopenic purpura
- Other side effects such as headache, back pain, dyspnea, nausea, hypertension, bradycardia, rash
Pregnancy & Lactation
- No adequate and well-controlled studies in pregnant women
- Use during nursing only if potential benefit justifies potential risk to nursing infant
Precautions & Warnings
- Risk of CABG-related bleeding
- Premature discontinuation increases risk of stent thrombosis, MI, and death
Use in Special Populations
- No established safety and effectiveness in pediatric patients
- Geriatric patients > 75 years at increased risk of fatal bleeding events
- No dosage adjustment necessary for renal impairment
- No dosage adjustment necessary for mild to moderate hepatic impairment
Overdose Effects
- Lethality observed in rats at high doses
- Platelet transfusion may restore clotting ability
