Indications
- Treatment of hypertension
- May be used alone or in combination with other antihypertensive agents
Pharmacology
- Blocks vasoconstrictor effects of angiotensin II
- Selective binding of angiotensin II to the AT 1 receptor in vascular smooth muscle
- Does not inhibit ACE (kininase II)
- No effect on response to bradykinin
Dosage & Administration
- Usual recommended starting dose of 20 mg once daily
- Can be increased to 40 mg for further reduction in blood pressure
- No advantage of twice-daily dosing over once daily
- No initial dosage adjustment for elderly patients or patients with renal or hepatic impairment
Interaction
- No significant drug interactions with digoxin or warfarin
- Bioavailability not altered by antacids
- Not metabolized by cytochrome P450 system
- Potential deterioration of renal function when co-administered with NSAIDs
Contraindications
- Hypersensitivity to any component of the product
Side Effects
- Well-tolerated with similar incidence of adverse reactions as placebo
- No dose-related frequency of adverse reactions
- Most common adverse reaction: dizziness
Pregnancy & Lactation
- Discontinue product when pregnancy is detected
- Potential adverse effects on nursing infant
Precautions & Warnings
- Changes in renal function may be anticipated in susceptible individuals
- Association with oliguria, progressive azotemia, acute renal failure, and death
Overdose Effects
- No experience of overdose reported
- Likely effects: hypotension and tachycardia
- Potential treatment includes gastric lavage or induction of emesis
