Indications

  • Reduces thrombotic cardiovascular events
  • Management of acute coronary syndrome

  • Primary PCI

Pharmacology

  • Inhibitor of platelet activation and aggregation
  • Irreversible binding to P2Y12 class of ADP receptors

Dosage & Administration

  • Initiated with a single 60 mg oral loading dose
  • Continuation at 10 mg once daily
  • Consider 5 mg once daily for patients <60 kg
  • Requires daily intake of aspirin

Interaction

  • Increased risk of bleeding with warfarin coadministration
  • Increased risk of bleeding with NSAIDs coadministration
  • Compatible with drugs that are inducers or inhibitors of cytochrome P450 enzymes
  • Compatible with aspirin, heparin, GPIIb/IIIa inhibitors, statins
  • Compatible with drugs that elevate gastric pH

Contraindications

  • Active pathological bleeding
  • History of prior transient ischemic attack or stroke

Pregnancy & Lactation

  • No adequate and well-controlled studies in pregnant women
  • Unknown excretion in human milk

Precautions & Warnings

  • Risk increases in patients receiving Prasurel who undergo CABG
  • Premature discontinuation increases risk of stent thrombosis, MI, and death

Use in Special Populations

  • Safety and effectiveness in pediatric patients not established
  • Increased risk of fatal bleeding events in patients above 75 years
  • No dosage adjustment necessary for patients with renal impairment
  • Limited experience in patients with end-stage renal disease
  • No dosage adjustment necessary in patients with mild to moderate hepatic impairment

Overdose Effects

  • Lethality observed in rats at 2000 mg/kg
  • Platelet transfusion may restore clotting ability
  • Active metabolite not likely to be removed by dialysis
