Indications
- Reduces thrombotic cardiovascular events
- Indicated for acute coronary syndrome
- Managed with percutaneous coronary intervention
- Effective for unstable angina (UA)
- Effective for non-ST-elevation myocardial infarction (NSTEMI)
- Effective for ST-elevation myocardial infarction (STEMI)
Pharmacology
- Platelet activation inhibitor
- Irreversible binding to P2Y12 class of ADP receptors
Dosage & Administration
- Initiate with 60 mg oral loading dose
- Maintain at 10 mg once daily
- Consider 5 mg once daily for patients <60 kg
- Should be taken with or without food
- Concomitant aspirin daily (75 mg to 325 mg)
Interactions
- Increased bleeding risk with warfarin coadministration
- Increased bleeding risk with NSAIDs (chronic use)
- Compatible with cytochrome P450 inducers or inhibitors
- Can be administered with aspirin, heparin, GPIIb/IIIa inhibitors, statins, drugs that elevate gastric pH
Contraindications
- Active pathological bleeding
- History of prior transient ischemic attack or stroke
Side Effects
- Bleeding
- Thrombotic thrombocytopenic purpura
- Other side effects: headache, back pain, dyspnea, nausea, hypertension, bradycardia, rash
Pregnancy & Lactation
- No adequate and well-controlled studies in pregnant women
- Not known if excreted in human milk
- Use during nursing only if potential benefit justifies potential risk to infant
Precautions & Warnings
- CABG-related bleeding risk
- Premature discontinuation increases risk of stent thrombosis, MI, and death
Use in Special Populations
- Pediatric Use (safety and effectiveness not established)
- Geriatric Use (increased risk of fatal bleeding events in patients >75 years)
- Renal Impairment (limited experience in patients with end-stage renal disease)
- Hepatic Impairment (no dosage adjustment for mild to moderate impairment)
Overdose Effects
- Lethality observed in rats at 2000 mg/kg
- Platelet transfusion may restore clotting ability
