Indications
- Reduce rate of thrombotic cardiovascular events in ACS patients
- Manage PCI for UA, NSTEMI, and STEMI patients
Pharmacology
- Inhibitor of platelet activation and aggregation
- Binds irreversibly to P2Y12 class of ADP receptors
Dosage & Administration
- Loading dose: 60 mg oral
- Maintenance dose: 10 mg daily
- Consider 5 mg for patients <60 kg
- Take with or without food
- Co-administer with daily aspirin (75 mg to 325 mg)
Interaction
- Increased bleeding risk with warfarin
- Potential bleeding risk with NSAIDs
- Can be administered with certain drugs and inhibitors/inducers of cytochrome P450 enzymes
Contraindications
- Active pathological bleeding
- History of TIA or stroke
Side Effects
- Bleeding
- Thrombotic thrombocytopenic purpura
- Other side effects: headache, back pain, dyspnea, nausea, hypertension, bradycardia, rash, etc.
Pregnancy & Lactation
- No adequate and well-controlled studies in pregnant women
- Use during nursing if potential benefit justifies potential risk to nursing infant
Precautions & Warnings
- Increased risk of CABG-related bleeding
- Premature discontinuation increases risk of stent thrombosis, MI, and death
Use in Special Populations
- Pediatric use not established for safety and effectiveness
- Increased risk of fatal bleeding events in patients >75 years old
- No dosage adjustment for renal impairment
- Limited experience in patients with end-stage renal disease
- No dosage adjustment for mild to moderate hepatic impairment
Overdose Effects
- Lethality observed in rats after 2000 mg/kg dose
- Platelet transfusion may restore clotting ability
- Active metabolite not likely to be removed by dialysis
