Medicine Details

Indications

  • Osteoarthritis
  • Rheumatoid arthritis
  • Ankylosing spondylitis
  • Decrease risk of NSAID-associated gastric ulcers

Pharmacology

  • Immediate release Esomeprazole Magnesium layer
  • Enteric-coated Naproxen core
  • Inhibition of prostaglandin synthesis
  • Proton pump inhibitor
  • Specific inhibition of the H+/k+ -ATPase
  • Reduction of gastric acidity

Dosage

  • Use lowest effective dose
  • Adult dosage: 375mg naproxen/20mg esomeprazole or 500mg naproxen/20mg esomeprazole twice daily
  • Adolescent dosage: Based on weight

Administration

  • Take at least 30 minutes before meals
  • Do not split, chew, crush, or dissolve the tablet

Interaction

  • Reduction of antihypertensive effect of ACE inhibitors, diuretics & beta-blockers
  • Increased risk of bleeding complication with warfarin
  • Interference with absorption of certain drugs
  • Effect of high-fat food on absorption

Contraindications

  • Hypersensitivity to any component of the tablet or substituted benzimidazoles
  • History of allergic-type reactions to aspirin or other NSAIDs
  • Use during peri-operative period in the setting of CABG surgery

Side Effects

  • Decreased occurrence of gastrointestinal side effects from Naproxen
  • Decrease in gastric ulcers and NSAID associated upper gastrointestinal adverse events
  • Risk of arterial thrombotic events
  • Gastrointestinal adverse events
  • Oedema, hypertension, cardiac failure
  • Gastrointestinal bleeding, ulceration, perforation, abdominal symptoms

Pregnancy & Lactation

  • Avoid use during first and second trimester of pregnancy
  • Contraindicated during the third trimester of pregnancy
  • Not recommended during breastfeeding
  • NSAIDs may impair female fertility

Precautions & Warnings

  • Avoid use with other NSAIDs and cyclooxygenase-2 inhibitors
  • Use with low dose acetylsalicylic acid
  • Minimize undesirable effects by using lowest effective dose
  • Risk-factors for NSAID related gastro-intestinal complications
  • Increased frequency of adverse reactions in older people
  • Gastrointestinal, cardiovascular, cerebrovascular, and renal effects

Use in Special Populations

  • Caution in elderly patients
  • Not recommended in patients with moderate to severe renal impairment
  • Monitoring in patients with mild to moderate hepatic impairment
