Indications
- Relief of signs and symptoms of osteoarthritis
- Relief of signs and symptoms of rheumatoid arthritis
- Relief of signs and symptoms of ankylosing spondylitis
- Decrease the risk of developing gastric ulcers in patients at risk of developing NSAID-associated gastric ulcers
Pharmacology
- Immediate release Esomeprazole Magnesium layer
- Enteric-coated Naproxen core
- Naproxen is a NSAID with analgesic & antipyretic properties
- Esomeprazole is a proton pump inhibitor that suppresses gastric acid secretion
Dosage
- Use the lowest effective dose
- One tablet twice daily for adults
- Different doses for Juvenile Idiopathic Arthritis in Adolescent Patients based on weight
Administration
- To be taken at least 30 minutes before meals
- Do not split, chew, crush, or dissolve the tablet
Interaction
- Concomitant use of NSAIDs may reduce the antihypertensive effect of ACE inhibitors, diuretics & beta-blockers
- Concomitant use with warfarin may result in an increased risk of a bleeding complication
- Esomeprazole may interfere with the absorption of drugs where gastric pH is an important determinant of bioavailability
Contraindications
- Known hypersensitivity to any component
- History of asthmay urticaria or other allergic-type reactions after taking aspirin or other NSAIDs
- Use during the peri-operative period in the setting of CABG surgery
Side Effects
- Decrease the occurrence of gastric ulcers and NSAID-associated upper gastrointestinal adverse events
- Increased risk of arterial thrombotic events with high doses and long-term treatment
- Gastrointestinal events like peptic ulcers, perforation or GI bleeding
- Other gastrointestinal side effects like nausea, vomiting, diarrhea, etc.
Pregnancy & Lactation
- Not recommended during the first and second trimester of pregnancy
- Contraindicated during the third trimester of pregnancy
- Not recommended during breastfeeding
- NSAIDs may impair female fertility
Precautions & Warnings
- Avoid combining with other NSAIDs
- Used with low dose acetylsalicylic acid
- Increased risk of gastrointestinal infections with proton pump inhibitors
- May reduce the absorption of vitamin B12
- Appropriate monitoring for patients with hypertension, heart failure, renal impairments, etc.
Use in Special Populations
- Use caution and dose adjustment in elderly patients
- Not recommended for use in patients with moderate to severe or severe renal impairment
- Close monitoring and possible dose reduction for patients with hepatic insufficiency
Overdose Effects
- Symptoms related to naproxen and esomeprazole overdose
- Management including symptomatic and supportive care, emesis, activated charcoal, and general supportive measures
