Pharmacology
- Inhibits platelet activation
- Irreversible binding to P2Y12 class of ADP receptors
- Dose-dependent inhibition of platelet aggregation
- Metabolized by the liver
- Rapid absorption after oral administration
- Linear pharmacokinetics
- Excreted in urine and faeces
- Half-life of main circulating metabolite: 8 hours
Dosage & Administration
- Recommended dose: 75 mg once daily
- Recommended dose for acute coronary syndrome: 300 mg loading dose followed by 75 mg once daily
- To be taken with or without food
- No dose adjustment for elderly patients or patients with renal impairment
- Aspirin should be initiated and continued in combination with Clopidogrel
Interactions
- No modification of Clopidogrel-mediated inhibition by Aspirin
- Potentiates effect of Aspirin on collagen-induced platelet aggregation
- No modification of heparin dose by Clopidogrel
- Caution with co-administration of NSAIDs
- Caution with co-administration of warfarin
Contraindications
- Hypersensitivity to the drug substance
- Active pathological bleeding
Side Effects
- Generally well tolerated
Pregnancy & Lactation
- Use during first and second trimesters if clearly needed
- Contraindicated during third trimester
- Unknown excretion in human breast milk
Precautions & Warnings
- May prolong bleeding time
- Rare reports of Thrombotic thrombocytopenic purpura (TTP)
- Risk of Reye's syndrome
- Hypersensitivity reactions reported
Use in Special Populations
- Not recommended for children under 12 years
- Caution with use in children due to Reye’s syndrome
Overdose Effects
- May lead to bleeding complications
- Platelet transfusion may restore clotting ability
- Aspirin overdose symptoms
