Indications
- Treatment of hypertension
- May be used alone or in combination with other antihypertensive agents
Pharmacology
- Blocks vasoconstrictor effects of angiotensin II
- Selective blocking of binding of angiotensin II to the AT 1 receptor
- Does not inhibit ACE or affect response to bradykinin
Dosage & Administration
- Usual recommended starting dose of 20 mg once daily
- Dose may be increased to 40 mg if further reduction in blood pressure is needed
- No advantage in twice-daily dosing
- No initial dosage adjustment for elderly patients or patients with renal or hepatic impairment
- Consideration for use of lower starting dose in patients with possible intravascular volume depletion
Interactions
- No significant drug interactions with digoxin or warfarin reported
- Does not affect cytochrome P450 system
- Potential renal function deterioration with NSAIDs and angiotensin II receptor antagonists
Contraindications
- Contraindicated in patients hypersensitive to any component of the product
Side Effects
- Well tolerated with similar incidence of adverse reactions to placebo
- Common adverse reactions include dizziness, back pain, diarrhea, headache, hyperglycemia, cough
- Minimal withdrawals due to adverse reactions
Pregnancy & Lactation
- Discontinue use when pregnancy is detected
- Potential adverse effects on nursing infant
Precautions & Warnings
- Changes in renal function may be anticipated
- Association with oliguria, progressive azotemia, acute renal failure, and death in susceptible individuals
Overdose Effects
- Hypotension and tachycardia are likely effects of overdose
- Clinically significant hypotension requires active cardiovascular system support
