Indications

  • Essential hypertension
  • Chronic congestive heart failure
  • Hepatic cirrhosis
  • Swelling (edema)
  • Hyperaldosteronism

Mode of Action

  • Complementary mechanisms
  • Additive or synergistic diuretic effect
  • Inhibition of Na/K/2Cl co-transporter
  • Blockage of reabsorption of sodium, potassium, and chloride ions
  • Antagonizing the action of aldosterone

Dosage & Administration

  • Daily dosage range of Furosemide 20-80 mg and Spironolactone 50-200 mg
  • Furosemide 40-80 mg and Spironolactone 50-100 mg for stabilized patients
  • Not suitable for use in children
  • Slower excretion in the elderly

Interaction

  • Increased risk of hyperkalemia with ACE inhibitors or potassium salts
  • Increased levels of cardiac glycosides in the blood
  • Reduced diuretic effects when used with corticosteroids or NSAIDs
  • Increased ototoxicity of aminoglycoside antibiotics
  • Reduced natriuretic and anti-hypertensive effect when used with sucralfate

Contraindications

  • Anuria
  • Acute renal insufficiency
  • Hyperkalaemia
  • Addison's disease
  • Hypersensitivity to Spironolactone, Furosemide, or sulphonamides

Precautions & Warnings

  • Caution in patients prone to electrolyte deficiency
  • Use with caution in diabetes, enlarged prostate, hypotension, and hypovolemia
