Indications
- Treatment of chronic hepatitis B virus infection in adults with compensated liver disease
Pharmacology
- Tenofovir alafenamide is a nucleoside analog reverse transcriptase inhibitor
- Converted to tenofovir through hydrolysis and subsequently phosphorylated by cellular kinases to the pharmacologically active metabolite tenofovir diphosphate
- Inhibits HBV replication through incorporation into viral DNA by the HBV reverse transcriptase
Dosage & Administration
- Adult dosage of 25 mg (one tablet) taken orally once daily with food
- No dosage adjustment required in patients with estimated creatinine clearance greater than or equal to 15 ml per minute
Interaction
- Substrate of P-glycoprotein (P-gp) and BCRP
- Drugs affecting renal function may increase concentrations of tenofovir and other renally eliminated drugs
Side Effects
- Common side effects include headache, stomach pain, tiredness, cough, nausea, back pain
Pregnancy & Lactation
- 4.2% prevalence of birth defects in live births following first trimester exposure
- Unknown presence of Tenofovir alafenamide & metabolites in human breast milk
Precautions & Warnings
- Discontinuation of treatment may result in severe acute exacerbations of hepatitis B
- Risk of development of HIV-1 resistance in patients coinfected with HBV and HIV-1
- Cases of renal impairment reported with TAF-containing products
Overdose Effects
- Monitor for evidence of toxicity
- Efficiently removed by hemodialysis with an extraction coefficient of approximately 54%
