Indications
- Treatment of gestational choriocarcinoma
- Treatment of acute lymphoblastic leukemia
- Treatment of non-Hodgkin lymphomas
- Management of severe rheumatoid arthritis
- Symptomatic control of severe psoriasis
Pharmacology
Inhibits dihydrofolic acid reductase, interferes with DNA synthesis, repair, and cellular replication, more sensitive to malignant cells
Dosage
- Choriocarcinoma and similar trophoblastic diseases: 15-30 mg daily for a five-day course, repeated 3-5 times
- Acute Lymphoblastic Leukemia:
- Induction dose: 3.3 mg/m2 in combination with prednisone 60 mg/m2 daily for 4-6 weeks
- Maintenance dose: 30 mg/m2 weekly
- Lymphoma: Variable dosage depending on tumor type and stage
- Breast Cancer: 40 mg/m2 intravenously on specific days in combination with other drugs
- Rheumatoid Arthritis: Single doses of 7.5-20 mg weekly
- Polyarticular-Course Juvenile Rheumatoid Arthritis: 10 mg/m2 once weekly
- Psoriasis: 10-30 mg once weekly
Administration
Best taken on an empty stomach, may be taken with meals to reduce GI discomfort, avoid taking with milk-rich products
Interaction
- NSAIDs: Should not be administered prior to or concomitantly with high doses
- Salicylates, Phenylbutazone, Phenytoin and Sulfonamides: Increased toxicity
- Cisplatin: Caution must be exercised if administered with high-dose Trexall
- Mercaptopurine: Increases the plasma levels
- Vitamin preparations containing folic acid or its derivatives: Decreases responses to systemically administered Trexall
Contraindications
- Alcoholism or chronic liver disease
- Immunodeficiency syndromes
- Pre-existing blood dyscrasias
- Hypersensitivity to methotrexate
- Pregnant or nursing women

Pregnancy & Lactation
US FDA Pregnancy Category X, contraindicated in nursing mothers
Precautions & Warnings
- Trexall formulations and diluents containing preservatives must not be used for intrathecal or high dose therapy
- Follow patients closely for toxic effects
- Reduce dosage or discontinue if adverse reactions occur
- Persistent liver function test abnormalities may be indicators of serious liver toxicity
Use in Special Populations
- Pediatric patients: Weigh benefits against potential risk
- Geriatric patients: Closely monitor for early signs of hepatic, bone marrow, and renal toxicity
Overdose Effects
Leucovorin is indicated to diminish the toxicity and counteract the effect of inadvertently administered overdosages
