Indications
- Reduce the rate of Myocardial Infarction (MI) and Stroke in patients with non-ST-segment elevation ACS
- Reduce the rate of MI and Stroke in patients with established peripheral arterial disease or history of recent MI or recent Stroke
Pharmacology
- Clopidogrel inhibits platelet activation and aggregation through irreversible binding of its active metabolite to P2Y12 class of ADP receptors on platelets
- Aspirin inhibits platelet aggregation by irreversible inhibition of platelet cyclooxygenase and inhibiting the generation of thromboxane A2
- Clopidogrel is extensively metabolized by the liver
- Clopidogrel undergoes rapid hydrolysis into its carboxylic acid derivative
Dosage & Administration
- Recommended dose - 75 mg once daily with or without food
- Initiate with a single 300 mg loading dose for acute coronary syndrome, then continue at 75 mg once daily
- Initiate and continue Aspirin in combination with Clopidogrel
Interaction
- Aspirin potentiates the effect of Clopidogrel on collagen-induced platelet aggregation
- Co-administration of heparin has no effect on inhibition of platelet aggregation induced by Clopidogrel
- NSAIDs and Clopidogrel should be co-administered with caution
Contraindications
- Hypersensitivity to the drug substance or any component of the product
- Active pathological bleeding such as peptic ulcer or intracranial hemorrhage
Side Effects
- Generally well tolerated
Pregnancy & Lactation
- Use during first and second trimesters of pregnancy only if clearly needed
- Contraindicated during the third trimester of pregnancy
- Unknown excretion of Clopidogrel in human breast milk, Aspirin is excreted in human milk
- Discontinue during breastfeeding
Precautions & Warnings
- May prolong bleeding time
- Reported cases of TTP and Reye's syndrome following use of Clontas
- Hypersensitivity reactions reported in patients receiving Clontas
Use in Special Populations
- Not recommended for children, particularly those under 12 years, unless benefits outweigh risks
- Aspirin may be a contributory factor in the causation of Reye's syndrome in some children
Overdose Effects
- Clopidogrel overdose may lead to bleeding complications
- Moderate aspirin intoxication - dizziness, headache, tinnitus, confusion, and gastrointestinal symptoms may occur
- Severe Aspirin intoxication - respiratory alkalosis, respiratory acidosis, metabolic acidosis, hyperthermia, perspiration, dehydration may occur
