Pharmacology

Voriconazole is a triazole antifungal medication that inhibits ergosterol synthesis by inhibiting CYP450-dependent 14-alpha sterol demethylase.

Dosage & Administration

  • Oral:
    • At or over 40 kg body weight:
      • Loading dose of 400 mg every 12 hours for the first 24 hours
      • Maintenance dose of 200 mg twice daily
    • Below 40 kg body weight:
      • Loading dose of 200 mg every 12 hours for the first 24 hours
      • Maintenance dose of 100 mg twice daily
  • Injection:
    • Invasive Aspergillosis:
      • Loading dose of 6 mg/kg 12 hourly for the first 24 hours
      • Maintenance dose of 4 mg/kg 12 hourly
    • Candidemia in nonneutropenic patients and other deep tissue Candida infections:
      • Loading dose of 6 mg/kg 12 hourly for the first 24 hours
      • Maintenance dose of 3-4 mg/kg 12 hourly
    • Scedosporiosis and Fusariosis:
      • Loading dose of 6 mg/kg 12 hourly for the first 24 hours
      • Maintenance dose of 4 mg/kg 12 hourly

Interaction

  • Interacts with CYP3A4, CYP2C9, and CYP2C19 inhibitors and inducers
  • May increase the concentrations and activity of drugs that are CYP3A4, CYP2C9, and CYP2C19 substrates
  • Increased maintenance oral and intravenous dosage with co-administration of Phenytoin or Efavirenz

Contraindications

  • Known hypersensitivity to Voriconazole or any other components:
    • Terfenadine
    • Astemizole
    • Cisapride
    • Pimozide
    • Quinidine
    • Sirolimus
    • Rifampin
    • Carbamazepine
    • Long-acting barbiturates
    • Efavirenz
    • Ritonavir
    • Rifabutin
    • Ergot alkaloids
    • St. John's Wort

Pregnancy & Lactation

No adequate and well-controlled studies in pregnant women. Use during pregnancy only if potential benefit justifies the potential risk to the fetus.

Precautions & Warnings

Long-term exposure requires careful assessment. Squamous cell carcinoma of the skin reported with long-term Viera treatment.

Use in Special Populations

Reconstitution

Shake the bottle well before adding water to loosen the powder. Add 25 ml of boiled and cooled water to the bottle and shake vigorously. Store reconstituted suspension between 15°C-30°C. Discard suspension 14 days after reconstitution.

Storage Conditions

Keep out of reach of children. Store in a dry place below 25°C temperature and protected from light. Store Viera powder for suspension between 2°C-8°C temperature.
