Description
- A tablet indicated for reducing the rate of myocardial infarction (MI) and stroke in patients with acute coronary syndrome (ACS), recent MI, recent stroke, or established peripheral arterial disease
- Contains the prodrug Clopidogrel which inhibits platelet activation and aggregation, and aspirin which inhibits platelet aggregation by irreversible inhibition of platelet cyclooxygenase
- Recommended dosage is 75 mg once daily, with or without food
- Combination contraindicated in conditions like hypersensitivity to the drug substance, active pathological bleeding such as peptic ulcer or intracranial hemorrhage
- Should be used with caution in pregnant women and during breastfeeding
- May prolong bleeding time and has been associated with TTP and Reye's syndrome
- Not recommended for children under 12 years

Pharmacological Action
- Inhibits platelet activation and aggregation through irreversible binding to the P2Y12 class of ADP receptors on platelets
- Inhibits platelet aggregation by irreversible inhibition of platelet cyclooxygenase and inhibiting the generation of thromboxane A2
Mode of Action
- Dose-dependent inhibition of platelet aggregation observed at 2 hours after single oral doses
- Repeated doses of 75 mg per day inhibit ADP-induced platelet aggregation on the first day
- Inhibition reaches steady state between Day 3 and Day 7
- Plasma concentrations of the parent compound are very low and generally below the quantification limit beyond 2 hours after dosing
- Main circulating metabolite is the carboxylic acid derivative, representing about 85% of the circulating drug-related compounds in plasma
- Approximately 50% excreted in the urine and approximately 46% in the faeces in the 5 days after dosing
- Elimination half-life of the main circulating metabolite is 8 hours after single and repeated administration
- Clopidogrel is rapidly absorbed after oral administration of repeated doses
- Peak plasma levels of the main circulating metabolite occur approximately 1 hour after dosing
- Main circulating metabolite binds reversibly in vitro to human plasma proteins (94%)
Dosage and Administration
- Recommended dose is 75 mg once daily with or without food
- No dosage adjustment necessary for elderly patients or patients with renal impairment
- For patients with acute coronary syndrome, Clopidogrel should be initiated with a single 300 mg loading dose and continued at 75 mg once daily
- Aspirin (75 mg-325 mg once daily) should be initiated and continued in combination with Clopidogrel
- Most patients with acute coronary syndrome also received heparin acutely
Specific Drug Interactions
- Aspirin does not modify the Clopidogrel-mediated inhibition of ADP-induced platelet aggregation
- Clopidogrel potentiates the effect of aspirin on collagen-induced platelet aggregation
- Clopidogrel does not alter the effect of heparin on coagulation
- Concomitant administration of Clopidogrel is associated with increased occult gastrointestinal blood loss when co-administered with NSAIDs
Contraindications
- Hypersensitivity to the drug substance or any component of the product
- Active pathological bleeding such as peptic ulcer or intracranial hemorrhage
Side Effects
- Generally well tolerated
Pregnancy and Lactation
- No adequate and well-controlled studies in pregnant women
- Use during first and second trimesters of pregnancy only if clearly needed
- Contraindicated during the third trimester of pregnancy
- Unknown whether Clopidogrel is excreted in human breast milk, aspirin is known to be excreted in human milk
- Drug should be discontinued during breastfeeding
Precautions and Warnings
- May prolong bleeding time
- TTP has been reported rarely following use of Clorel-A
- Reye's syndrome may develop in individuals who have chicken pox, influenza or flu symptoms
- Hypersensitivity, including rash, angioedema, or hematologic reaction, has been reported in patients receiving Clorel-A
Use in Special Populations
- Not recommended for children, particularly those under 12 years
Overdose Effects
- Clopidogrel overdose may lead to bleeding complications
- Moderate aspirin intoxication symptoms can be treated by inducing vomiting followed by gastric lavage if needed
- Severe aspirin intoxication symptoms can be treated with hemodialysis and other symptomatic treatment
