Indications
- Reduction of Myocardial Infarction (MI) and Stroke in patients with non-ST-segment elevation ACS
- Reduction of MI and Stroke in patients with acute ST-segment elevation ACS
- Reduction of MI and Stroke in patients with recent MI, recent Stroke, or established Peripheral Arterial Disease
Pharmacology
- Clopidogrel is a prodrug
- Inhibits platelet activation and aggregation
- Irreversible binding of active metabolite to the P2Y12 class of ADP receptors
- Aspirin inhibits platelet aggregation by irreversible inhibition of platelet cyclooxygenase
- Inhibition of thromboxane A2 generation
- Pharmacokinetics include plasma concentrations of the parent compound, metabolism, and elimination
Dosage & Administration
- Recommended dose of 75 mg once daily with or without food
- Initiation with a single 300 mg loading dose for patients with acute coronary syndrome
- Combination with Aspirin
- Most patients with acute coronary syndrome also received heparin acutely
Interaction
- Interactions with Aspirin, Heparin, Nonsteroidal Anti-Inflammatory Drugs (NSAIDs), and Warfarin
Contraindications
- Hypersensitivity to the drug substance or any component of the product
- Active pathological bleeding such as peptic ulcer or intracranial hemorrhage
Side Effects
- Generally well tolerated
Pregnancy & Lactation
- Limited studies in pregnant women
- Usage during first and second trimesters if clearly needed
- Contraindicated during the third trimester
- Unknown excretion in human breast milk
- Aspirin excreted in human milk
Precautions & Warnings
- Prolongs the bleeding time
- Rare reports of Thrombotic thrombocytopenic purpura (TTP) and Reye's syndrome
- Hypersensitivity reactions reported
Use in Special Populations
- Not recommended for children under 12 years, unless benefits outweigh risks
- Aspirin may be a contributory factor in Reye's syndrome in some children
Overdose Effects
- May lead to bleeding complications
- Platelet transfusion may restore clotting ability
- Symptoms and treatment of moderate and severe aspirin intoxication
