Indications
- Treatment of advanced renal cell carcinoma (RCC)
- Treatment of hepatocellular carcinoma (HCC) after Sorafenib treatment
Pharmacology
- Inhibition of tyrosine kinase activity of MET, VEGFR-1, -2 and -3, AXL, RET, ROS1, TYRO3, MER, KIT, TRKB, FLT-3, TIE-2
- Receptor tyrosine kinases involved in cellular function, oncogenesis, metastasis, tumor angiogenesis, drug resistance
- Absorption within 3-4 hours post-dose
- Distribution with high protein binding in human plasma
- Predicted terminal half-life of approximately 99 hours
- Metabolism as a substrate of CYP3A4 in vitro
- Excretion of approximately 54% in feces and 27% in urine
Dosage & Administration
- Recommended dosage of 60 mg once daily without food for renal cell carcinoma
- Recommended dosage of 60 mg once daily without food for hepatocellular carcinoma
- Dosage adjustment for patients with hepatic impairment and those taking CYP450 inhibitors/inducers
- Precautions for administration near scheduled surgery, capsule substitution, food intake, and missed doses
Interaction
- Increased exposure and risk of adverse reactions with strong CYP3A4 inhibitors
- Decreased exposure and reduced efficacy with strong CYP3A4 inducers
- Avoidance of coadministration with grapefruit or grapefruit juice, and St. John's Wort
Contraindications
- Contraindicated in patients with known hypersensitivity to Cabozantinib or any other components

Pregnancy & Lactation
- Potential fetal harm during pregnancy
- Advisement against breastfeeding during treatment and for 4 months after the final dose
- Requirement for effective contraception in females of reproductive potential
- Potential impairment of fertility in females and males based on animal studies
Precautions & Warnings
- Discontinuation for severe hemorrhage, recent history of hemorrhage, and signs of fistulas and perforations
- Risk of thrombotic events and need for regular blood pressure monitoring
- Management of diarrhea, palmar-plantar erythrodysesthesia, proteinuria, osteonecrosis of the jaw, wound complications, and reversible posterior leukoencephalopathy syndrome
- Consideration of embryo-fetal toxicity and associated risks
Overdose Effects
- Reported case involving memory impairment, mental status changes, cognitive disturbance, weight loss, and increase in BUN
