Indications
- Reduction of rate of Myocardial Infarction and Stroke in patients with ACS
- Reduction of rate of MI and Stroke in patients with recent MI, recent Stroke, or established PAD
Pharmacology
- Clopidogrel inhibits platelet activation and aggregation
- Aspirin inhibits platelet aggregation by irreversible inhibition of platelet cyclooxygenase
Pharmacokinetics
- Rapid absorption after oral administration
- Extensively metabolized by the liver
Dosage & Administration
- Recommended dose of 75 mg once daily
- No dosage adjustment for elderly patients or patients with renal impairment
Interaction
- Clopidogrel does not necessitate modification of heparin dose
- Increased occult gastrointestinal blood loss with NSAIDs
Contraindications
- Hypersensitivity to the drug substance
- Active pathological bleeding such as peptic ulcer
Side Effects
- Generally well tolerated
Pregnancy & Lactation
- Use during first and second trimesters only if clearly needed
- Contraindicated during the third trimester
- Unknown excretion in human breast milk
Precautions & Warnings
- May prolong bleeding time
- Rare reports of Thrombotic thrombocytopenic purpura (TTP)
Use in Special Populations
- Not recommended for children under 12 years
- Benefit-risk ratio should be considered
