Indications

Reduce rate of Myocardial Infarction (MI)

Pharmacology

Clopidogrel is a prodrug
Inhibits platelet activation and aggregation
Irreversible binding to P2Y12 class of ADP receptors
Dose-dependent inhibition of platelet aggregation
Aspirin inhibits platelet aggregation by irreversible inhibition of platelet cyclooxygenase

Dosage & Administration

Recommended dose of 75 mg once daily
Initial 300 mg loading dose for acute coronary syndrome
Can be taken with or without food
Combination therapy with Aspirin
Studies conducted with heparin

Interaction

No modification of Clopidogrel by Aspirin
Increased gastrointestinal blood loss with NSAIDs
Concomitant use caution with Warfarin

Contraindications

Hypersensitivity to drug substance
Active pathological bleeding

Side Effects

Generally well tolerated

Pregnancy & Lactation

Use only if clearly needed in first and second trimesters
Contraindicated in third trimester
Unknown excretion in breast milk
Discontinue during breastfeeding

Precautions & Warnings

May prolong bleeding time
Rare reports of TTP
Development of Reye's syndrome
History of hypersensitivity reactions

Use in Special Populations

Not recommended for children under 12
Caution in benefits vs risks assessment

Overdose Effects

May lead to bleeding complications
Symptoms include dizziness, headache, confusion
Treatment involves platelet transfusion
Severe cases may require haemodialysis
