Indications

  • Treatment of chronic hepatitis B virus infection in adults with compensated liver disease

Pharmacology

  • Tenofovir alafenamide is a phosphonamidate prodrug of tenofovir
  • Tenofovir alafenamide enters primary hepatocytes by passive diffusion
  • Converted to tenofovir through hydrolysis primarily by carboxylesterase 1 (CES1)
  • Intracellular tenofovir inhibits HBV replication through incorporation into viral DNA
  • Weak inhibitor of mammalian DNA polymerases

Dosage & Administration

  • Testing for HIV-1 infection prior to initiation
  • Recommended dosage of Tenofovir alafenamide is 25 mg once daily with food
  • No dosage adjustment in patients with estimated creatinine clearance greater than or equal to 15 ml per minute

Interaction

  • Telabee is a substrate of P-glycoprotein (P-gp) and BCRP
  • Drugs that strongly affect P-gp and BCRP activity may lead to changes in Telabee absorption
  • Coadministration with drugs that inhibit P-gp and BCRP may increase the absorption & plasma concentration

Side Effects

  • Common side effects include headache, stomach pain, tiredness, cough, nausea, back pain

Pregnancy & Lactation

  • Prevalence of birth defects in live births following exposure to TAF-containing regimens
  • Effect of Tenofovir alafenamide on breastfed infants should be considered

Precautions & Warnings

  • Discontinuation of treatment may result in severe acute exacerbations of hepatitis B
  • Telabee alone is not recommended for the treatment of HIV-1 infection
  • Postmarketing cases of renal impairment have been reported with TAF-containing products

Overdose Effects

  • Monitor patient for evidence of toxicity
  • Treatment of overdosage with Telabee consists of general supportive measures
  • Telabee is efficiently removed by hemodialysis
