Indications
- Reduces the rate of Myocardial Infarction (MI)
- Reduces the rate of Stroke
- Treatment for non-ST-segment elevation ACS
- Treatment for ST-segment elevation ACS
- Reduces the rate of MI in patients with Peripheral Arterial Disease
- Reduces the rate of Stroke in patients with Peripheral Arterial Disease
Pharmacology
- Inhibits platelet activation and aggregation
- Binds to P2Y12 class of ADP receptors
- Dose-dependent inhibition of platelet aggregation
- Irreversible inhibition of platelet cyclooxygenase
- Inhibits generation of thromboxane A2
- Rapid absorption after oral administration
- Extensively metabolized by the liver
- Plasma concentrations of the parent compound after repeated doses
- Plasma concentrations of the main circulating metabolite
- Binds reversibly to human plasma proteins
Dosage & Administration
- Recommended dose of 75 mg once daily
- Single 300 mg loading dose for acute coronary syndrome
- Combination therapy with Aspirin (75-325 mg once daily)
- No dosage adjustment necessary for elderly patients
- No dosage adjustment necessary for patients with renal impairment
Interaction
- Potentiates the effect of aspirin on collagen-induced platelet aggregation
- No effect on heparin dose or its effect on coagulation
- Association with increased occult gastrointestinal blood loss when co-administered with NSAIDs
- Caution when co-administered with Warfarin
Contraindications
- Hypersensitivity to the drug substance
- Hypersensitivity to any component of the product
- Active pathological bleeding such as peptic ulcer or intracranial hemorrhage
Side Effects
- Generally well tolerated
Pregnancy & Lactation
- Use during first and second trimesters if clearly needed
- Contraindicated during the third trimester
- Unknown excretion in human breast milk
- Known excretion of Aspirin in human milk
- Discontinue during breastfeeding
Precautions & Warnings
- May prolong bleeding time
- Reported cases of Thrombotic thrombocytopenic purpura (TTP)
- Risk of Reye's syndrome
- Reported hypersensitivity reactions in patients with history of hypersensitivity to other thienopyridines
Use in Special Populations
- Not recommended for children under 12 years
- Risks versus benefits assessment for use in children
- Aspirin may contribute to the causation of Reye’s syndrome in some children
Overdose Effects
- May lead to bleeding complications
- Treatment with platelet transfusion in case of overdose
- Symptomatic treatment for aspirin overdose
