Indications

  • Treatment of type 2 diabetes mellitus in overweight patients
  • Monotherapy or combination therapy for adults and children
  • Reducing diabetic complications in overweight type 2 diabetic adult patients

Pharmacology

Biguanide type oral antihyperglycemic drug for type 2 diabetes management

Dosage & Administration

  • Metformin immediate release tablet:
    • Usual starting dose for adults: 500 mg twice a day or 850 mg once a day
    • Maximum daily dose for adults: 2550 mg
    • Starting dose for children: 500 mg twice a day
    • Maximum daily dose for children: 2000 mg
  • Metformin extended release tablet:
    • Usual starting dose for adults: 500 mg once daily
    • Maximum daily dose for adults: 2000 mg once daily or 1000 mg twice daily
    • Not studied in children
  • Renal impaired patient: Avoid use in patients with eGFR below 30 mL/min/1.73 m^2

Interaction

  • Potential interaction with Furosemide and Nifedipine
  • Effect on cationic drugs eliminated by renal tubular secretion
  • No effect on Cimetidine pharmacokinetics
  • Drugs that may produce hyperglycemia

Contraindications

  • Hypersensitivity to active substance or excipients
  • Acute metabolic acidosis
  • Severe renal failure
  • Acute conditions altering renal function
  • Conditions causing tissue hypoxia

Side Effects

  • Hemolytic anemia
  • Lactic acidosis
  • Vitamin B12 absorption reduction
  • Peripheral neuropathy in patients with vitamin B12 deficiency
  • Taste disturbance
  • Encephalopathy
  • Gastrointestinal disorders
  • Liver function test abnormalities
  • Skin reactions

Pregnancy & Lactation

  • Pregnancy: Not recommended, insulin preferred for maintaining normal blood glucose levels
  • Breastfeeding: Excreted into human breast milk, limited data available

Precautions & Warnings

  • Substantial excretion by the kidney
  • Lowering of vitamin B12 level
  • Increased risk of hypoglycemia when used with insulin
  • Regular monitoring of renal function
  • Therapy cessation before certain clinical investigations and surgery
  • Caution in conditions causing dehydration or serious infections
  • Annual estimation of Vitamin B-12 levels
  • Blood glucose monitoring during combined therapy with a sulphonylurea
  • Reduced renal clearance during cimetidine therapy
  • Possible interaction with anticoagulants

Use in Special Populations

  • Elderly: Dosage adjustment based on renal function
  • Pediatric population: Diagnosis confirmation before initiation, no effect on growth and puberty detected
  • Children aged between 10 and 12 years: Caution recommended
  • Renal function: Creatinine clearance determination before and during treatment, contraindicated in patients with GFR < 30 ml/min

Overdose Effects

Hypoglycemia not seen with usual doses, lactic acidosis is a medical emergency
