Dosage & Administration

  • Initiation with a single 300 mg loading dose for patients with acute coronary syndrome (unstable angina/non-Q-wave MI)
  • Combination with Aspirin (75 mg-325 mg once daily)

Interaction

  • No modification by Aspirin of the Clopidogrel-mediated inhibition of ADP-induced platelet aggregation
  • Concomitant administration with NSAIDs associated with increased occult gastrointestinal blood loss
  • Caution with concomitant administration of Warfarin

Contraindications

  • Hypersensitivity to the drug substance or any component
  • Active pathological bleeding such as peptic ulcer or intracranial hemorrhage

Side Effects

  • Generally well tolerated

Pregnancy & Lactation

  • Use during first and second trimesters of pregnancy only if clearly needed
  • Contraindicated during the third trimester of pregnancy
  • Discontinuation during breastfeeding

Precautions & Warnings

  • May prolong bleeding time
  • Rare reports of Thrombotic thrombocytopenic purpura (TTP)
  • Potential development of Reye's syndrome

Use in Special Populations

  • Not recommended for children, particularly those under 12 years
  • Aspirin may be a contributory factor in the causation of Reye's syndrome in some children

Overdose Effects

  • Clopidogrel overdose may lead to bleeding complications
  • Moderate Aspirin intoxication symptoms include dizziness, headache, tinnitus, confusion, and gastrointestinal symptoms
  • Severe Aspirin intoxication may lead to respiratory and metabolic issues
