Indications
- Adjunct in the treatment of parkinsonism
- Useful as adjuvant therapy with levodopa
- Control of extrapyramidal disorders
Description
- Exerts inhibitory effect on parasympathetic nervous system
- Relaxing effect on smooth musculature
Pharmacology
- Non-selective muscarinic acetylcholine receptor antagonist
- Binds with higher affinity to M1 subtype
- Higher affinity for central muscarinic receptors in cerebral cortex
- May modify nicotinic acetylcholine receptor neurotransmission
Dosage & Administration
- Individualized dosage
- Low initial dose, gradual increase, especially in elderly patients
- May be given before or after meals
- Abrupt withdrawal may result in acute exacerbation of symptoms
- May be used concomitantly with levodopa or other parasympathetic inhibitors
Interaction
- Additive effects with cannabinoids, barbiturates, opiates, and alcohol
- Increased sedative effects with alcohol or other CNS depressants
- Increased risk for tardive dyskinesia with neuroleptics
- Possible need to reduce dosage of levodopa during concomitant therapy

- Hypersensitivity to Trihexyphenidyl
- Narrow angle glaucoma

Precautions & Warnings
- Gonioscope evaluation prior to therapy
- Monitoring of intraocular pressures
- Caution in hot weather, especially with concomitant use of other anticholinergics
- Risk of hyperthermia with anhidrosis
- Potential for Neuroleptic Malignant Syndrome
Overdose Effects
- Central symptoms of atropine intoxication
- Neuropsychiatric signs such as delirium, disorientation, seizures
- Treatment involves symptomatic and supportive therapy
