Indications
- Adjunct in the treatment of all forms of parkinsonism
- Adjuvant therapy for Parkinsonism with levodopa
- Control of extrapyramidal disorders caused by central nervous system drugs
Description
- Exerts a direct inhibitory effect upon the parasympathetic nervous system
- Relaxing effect on smooth musculature
Pharmacology
- Non-selective muscarinic acetylcholine receptor antagonist
- Higher affinity for central muscarinic receptors located in the cerebral cortex
- Lower affinity for peripheral muscarinic receptors
Dosage & Administration
- Individualized dosage
- Low initial dose with gradual increase
- Appropriate timing with meals
- Abrupt withdrawal may result in acute exacerbation of symptoms
- Dosage adjustment when used concomitantly with other drugs
Interaction
- Additive effects with cannabinoids, barbiturates, opiates, and alcohol
- Increased sedative effects with concurrent use of alcohol or other CNS depressants
- Increased risk for tardive dyskinesia with concomitant administration of neuroleptics
- Need for dosage reduction when used concomitantly with monoamine oxidase inhibitors or tricyclic antidepressants

- Narrow angle glaucoma

Precautions & Warnings
- Gonioscope evaluation and monitoring of intraocular pressures
- Risk of angle closure and increased intraocular pressure
- Caution in hot weather and in certain patient populations
- Potential occurrence of high anhidrosis and hyperthermia
- Neuroleptic Malignant Syndrome
Overdose Effects
- Typical central symptoms of atropine intoxication
- Neuropsychiatric signs such as delirium, anxiety, hallucinations
- Coma, paralysis, cardiac and respiratory arrest
- Gastric lavage for limiting absorption
- Supportive therapy for symptomatic treatment
