Indications
- Adjunct in the treatment of parkinsonism

Description
- Exerts inhibitory effect on parasympathetic nervous system
- Relaxing effect on smooth musculature
Pharmacology
- Non-selective muscarinic acetylcholine receptor antagonist
- Binds with higher affinity to M1 subtype
- Higher affinity for central muscarinic receptors in cerebral cortex
Dosage & Administration
- Individualized dosage
- Low initial dose
- Increased gradually, especially in patients over 60 years
- May be taken before or after meals
- Abrupt withdrawal may result in acute exacerbation of Parkinsonism symptoms
- Dosage adjustments for different forms of parkinsonism
Interaction
- Additive effects with cannabinoids, barbiturates, opiates, and alcohol
- Increased sedative effects with alcohol or other CNS depressants
- Intensified anticholinergic effects with monoamine oxidase inhibitors and tricyclic antidepressants
- Increased risk for tardive dyskinesia during concomitant administration of anticholinergics and neuroleptics

Precautions & Warnings
- Gonioscope evaluation prior to initiation of therapy
- Close monitoring of intraocular pressures
- Caution in hot weather
- Consideration of narrow angle glaucoma if blurring of vision occurs
- Possible hyperthermia with anhidrosis
- Severe anhidrosis and fatal hyperthermia under certain conditions
Overdose Effects
- Typical central symptoms of atropine intoxication
- Neuropsychiatric signs
- Progression to stupor, coma, paralysis, cardiac and respiratory arrest, and death
- Symptomatic and supportive therapy for acute overdose
- Gastric lavage or other methods to limit absorption
- Respiratory support, artificial respiration, or vasopressor agents
