Indications

  • Amoxicillin and Clavulanic Acid in 375 mg tablet
  • Amoxicillin and Clavulanic Acid in 625 mg tablet
  • Amoxicillin and Clavulanic Acid in 1 gm tablet
  • Amoxicillin and Clavulanic Acid in Powder for Suspension
  • Amoxicillin and Clavulanic Acid in Forte Powder for Suspension
  • Amoxicillin Sodium and Clavulanate Potassium in 1.2 gm Injection
  • Amoxicillin Sodium and Clavulanate Potassium in 0.6 gm Injection

Pharmacology

  • Bactericidal activity against Gram-positive & Gram-negative microorganisms
  • Broad spectrum of activity
  • Inactivation of beta-lactamase enzymes
  • Optimized absorption at the start of a meal
  • Low levels of serum binding for both clavulanate and Amoxicillin

Dosage

  • Adult dosage for tablet and suspension
  • Children dosage for suspension and forte suspension
  • IV injection dosage for adults and children
  • Increased dosage for severe infections
  • Dosage adjustment for renal impairment
  • Dosage caution for hepatic impairment

Administration

  • Oral dosage form can be taken without regard to meals
  • Enhanced absorption when administered at the start of a meal
  • IV injection for intravenous administration
  • IV injection should be used within 20 minutes of reconstitution
  • IV injection should not be mixed with certain fluids

Interaction

  • Prolongation of bleeding time and prothrombin time
  • Reduction of oral contraceptives efficacy
  • Concomitant use with allopurinol
  • No data on concomitant use with allopurinol

Contraindications

  • History of Penicillin hypersensitivity
  • Possible cross-sensitivity with other beta-lactam antibiotics
  • Contraindicated for patients with previous Co-amoxiclav or Penicillin-associated cholestatic jaundice

Side Effects

  • Uncommon side effects of mild and transitory nature
  • Gastrointestinal side effects may be reduced by taking with meals
  • Rare reports of hepatitis and cholestatic jaundice
  • Occasional occurrence of rashes and skin reactions
  • Reports of angioedema and anaphylaxis

Pregnancy & Lactation

  • No teratogenic effect in animal studies
  • Limited use in human pregnancy with no untoward effects
  • Not recommended in pregnancy unless essential
  • Trace quantities of Amoxicillin in breast milk during lactation

Precautions & Warnings

  • Use with care in patients on anticoagulation therapy
  • Dosage adjustment for severe hepatic dysfunction
  • Dosage adjustment for moderate or severe renal impairment
  • Maintenance of adequate fluid intake and urinary output during high dose administration

Use in Special Populations

  • Dosage adjustment for renal impairment in adults and children
  • Caution and monitoring for hepatic impairment
  • Additional IV dose may be needed during dialysis

Overdose Effects

  • Unlikely to occur
  • Possibility of gastrointestinal symptoms
  • Possibility of disturbance of fluid and electrolyte balances
  • Removal from circulation by haemodialysis

Reconstitution

  • Reconstitution of 1.2 gm IV injection with Water for Injection BP
  • Limitations on reconstitution or mixing with other solutions
  • Injection into infusion fluids over a period of 3-4 minutes

Storage Conditions

  • Storage below 25°C, protected from light and moisture
  • Refrigeration of reconstituted suspension (not frozen)
  • Reconstituted vial usage within 20 minutes
