Pharmacology

Prodrug
Inhibits platelet activation and aggregation
Irreversible binding to P2Y12 class of ADP receptors
Dose-dependent inhibition of platelet aggregation
Metabolism by the liver
Elimination half-life of the main circulating metabolite is 8 hours

Dosage & Administration

Recommended dose: 75 mg once daily
Loading dose: 300 mg for acute coronary syndrome
Administer with or without food
Combine with aspirin
Elderly and renal patients do not require dosage adjustment

Interaction

Clopidogrel potentiates the effect of aspirin
No effect on heparin's action
Increased occult gastrointestinal blood loss with NSAIDs
Caution with concomitant warfarin administration

Contraindications

Hypersensitivity to the drug substance
Active pathological bleeding

Pregnancy & Lactation

Use during first and second trimesters if clearly needed
Contraindicated during the third trimester
Unknown excretion in human breast milk

Precautions & Warnings

May prolong bleeding time
Rare reports of TTP
Possible development of Reye's syndrome
Hypersensitivity reactions

Use in Special Populations

Not recommended for children under 12 years
Caution with Reye's syndrome in children due to aspirin
