Indications
- Treatment of acute and chronic psychoses
- Treatment of schizophrenia
- Treatment of bipolar disorder
- Prevention of recurrence of manic, depressive, or mixed episodes
Pharmacology
- Belongs to dibenzothiazepine derivatives class
- Antagonist at serotonin, dopamine, histamine, and adrenergic receptors
- Unknown mechanism of action
- Efficacy in schizophrenia through dopamine and serotonin antagonism
Dosage & Administration
- Administered twice daily
- Titration to usual effective dose range
- Dosage adjustments for manic episodes and depressive episodes
- Combination therapy for maintenance treatment of bipolar I disorder
Interaction
- Caution with medicines causing electrolyte imbalance
- Co-administration with thioridazine, carbamazepine, phenytoin increases clearance
Contraindications
- Contra-indicated in patients hypersensitive to it
Side Effects
- Common adverse drug reactions like somnolence and dizziness
- Elevations in serum triglyceride and total cholesterol levels
- Weight gain and decreased haemoglobin
- Extrapyramidal symptoms

Precautions & Warnings
- Risk of suicidal thoughts and clinical worsening
- Caution in patients with cardiovascular or cerebrovascular disease
- Tardive dyskinesia and extrapyramidal symptoms
- Neuroleptic malignant syndrome
- QT prolongation
- Severe neutropenia reported
- Hyperglycemia and diabetes mellitus observed
Use in Special Populations
