Indications
- Symptomatic treatment of Alzheimer's dementia
- Symptomatic treatment of dementia in patients with Parkinson's disease
Pharmacology
- Carbamate derivative
- Structurally related to physostigmine
- Binds reversibly with and inactivates cholinesterase
- Increases concentration of acetylcholine at cholinergic synapses
- Relatively specific for brain acetylcholinesterase and butyrylcholinesterase
Dosage
- Initial dose of 1.5 mg twice a day for capsules
- Dose titration based on tolerability
- Maintenance dose of 3-6 mg twice a day
- Transdermal patch: 4.6 mg/24 hours initial dose
- Dose titration for transdermal patch
- Recommended maximum daily dose of 6 mg twice a day
Administration
- Administered twice a day with meals
Interaction
- Exaggerates effects of succinylcholine-type muscle relaxants
- Requires caution with anaesthetic agents
- Should not be given concomitantly with other cholinomimetic substances
- May interfere with anticholinergic medicinal products
Contraindications
- Contraindicated in patients with hypersensitivity to the active substance or other carbamate derivatives

Pregnancy & Lactation
- Not recommended during pregnancy
- Excreted into animal milk - not known if excreted into human milk

- Monitoring body weight is recommended
- May worsen extrapyramidal symptoms
