Indications
- Symptomatic treatment of mild to moderately severe Alzheimer's dementia
- Symptomatic treatment of mild to moderately severe dementia in patients with Idiopathic Parkinson's disease
Pharmacology
- Rivastigmine is a carbamate derivative
- Structurally related to physostigmine
- Binds reversibly with and inactivates cholinesterase
- Prevents the hydrolysis of acetylcholine
Dosage
- Initial dose: Rivastigmine 1.5 mg twice a day
- Dose titration based on tolerability
- Maintenance dose: 3 mg to 6 mg twice a day
- Maximum daily dose: 6 mg twice a day
- Apply patch on intact skin for a 24-hour period
Administration
- Administered twice a day, with morning and evening meals
Interaction
- Exaggerates effects of succinylcholine-type muscle relaxants during anaesthesia
- Caution with anaesthetic agents selection
- Should not be given concomitantly with other cholinomimetic substances
- Possible dose adjustments or temporarily stopping treatment
Contraindications
- Contraindicated in patients with hypersensitivity to the active substance or other carbamate derivatives
Side Effects
- Most commonly reported adverse reactions are gastrointestinal
- Nausea (38%) and vomiting (23%) especially during titration
- Female patients found to be more susceptible than male patients to gastrointestinal adverse reactions and weight loss
Pregnancy & Lactation
- Should not be used during pregnancy unless clearly necessary
- Excreted into milk; not known if excreted into human milk
- Should not breast-feed while on rivastigmine
Precautions & Warnings
- Cholinergic stimulation may increase gastric acid secretion
- May exacerbate urinary obstruction and seizures
- Monitor body weight
- May worsen extrapyramidal symptoms
Use in Special Populations
- Increased exposure in moderate renal and mild to moderate hepatic impairment
- Dosing recommendations to titrate according to individual tolerability should be closely followed
- Not recommended for use in children
Overdose Effects
- Most cases of accidental overdose have not been associated with any clinical signs or symptoms
- Symptoms may include nausea, vomiting, diarrhea, hypertension, hallucinations
- Vagotonic effect on heart rate; bradycardia and/or syncope may occur
- Asymptomatic overdose: no further dose of rivastigmine should be administered for the next 24 hours
