Indications
- Symptomatic treatment of Alzheimer's dementia
- Symptomatic treatment of dementia in patients with Idiopathic Parkinson's disease
Pharmacology
- Structurally related to physostigmine
- Binds reversibly with and inactivates cholinesterase
- Prevents hydrolysis of acetylcholine
- Increases concentration of acetylcholine at cholinergic synapses
- Relatively specific for brain acetylcholinesterase
- Relatively specific for brain butyrylcholinesterase
Dosage
- Initial dose: Rivastigmine 1.5 mg twice a day
- Maintenance dose: 3 mg to 6 mg twice a day
- Maximum daily dose: 6 mg twice a day
- Apply patch on intact skin for a 24-hour period
- Replace patch every 24 hours
Administration
- Administered twice a day, with morning and evening meals
Interaction
- May exaggerate the effects of succinylcholine-type muscle relaxants
- No pharmacokinetic interaction with digoxin, warfarin, diazepam, or fluoxetine
Contraindications
- Contraindicated in patients with hypersensitivity to the active substance or other carbamate derivatives
Side Effects
- Most commonly reported adverse reactions: nausea and vomiting
- Female patients found to be more susceptible than male patients
- Adverse reactions may include weight loss
Pregnancy & Lactation
- Should not be used during pregnancy unless clearly necessary
- Excreted into milk
- Should not be used while breastfeeding
Precautions & Warnings
- Increase in gastric acid secretion
- May exacerbate urinary obstruction and seizures
- Monitor body weight
- May worsen extrapyramidal symptoms
- Not recommended for use in children
- Monitor closely in renal and hepatic impairment

Overdose Effects
- Most cases of accidental overdose not associated with clinical signs or symptoms
- Symptoms may include nausea, vomiting, diarrhea, hypertension, and hallucinations
- Bradycardia and/or syncope may occur
- Ingestion of 46 mg in one case
- No further dose should be administered in asymptomatic overdose for the next 24 hours
