Indications
- Symptomatic treatment of mild to moderately severe Alzheimer's dementia
- Symptomatic treatment of mild to moderately severe dementia in patients with Idiopathic Parkinson's disease
Pharmacology
- Rivastigmine is a carbamate derivative that is structurally related to physostigmine
- Binds reversibly with and inactivates cholinesterase, preventing the hydrolysis of acetycholine
- Increases concentration of acetylcholine at cholinergic synapses
- Relatively specific for brain acetylcholinesterase and butyrylcholinesterase compared with those in peripheral tissues
Dosage
- Rivastigmine capsule initial dose: 1.5 mg twice a day
- Dose titration based on tolerability
- Maintenance dose: 3 mg to 6 mg twice a day
- Re-initiation of therapy after interruption for more than several days: 1.5 mg twice daily
- Rivastigmine transdermal patch initial dose: 4.6 mg/24 hours
- Dose titration after a minimum of 4 weeks
- Dosage for mild to moderate Alzheimer's Disease and Parkinson’s Disease Dementia
- Dosage for severe Alzheimer’s Disease
- Retitrate dosage after treatment interruption longer than 3 days
Administration
- Administered twice a day with morning and evening meals
Interaction
- Exaggerates the effects of succinylcholine-type muscle relaxants during anaesthesia
- Caution recommended in selecting anaesthetic agents
- Should not be given concomitantly with other cholinomimetic substances
- Might interfere with the activity of anticholinergic medicinal products
- No pharmacokinetic interaction observed with digoxin, warfarin, diazepam, or fluoxetine
Contraindications
- Contraindicated in patients with hypersensitivity to the active substance or other carbamate derivatives
Side Effects
- Most commonly reported adverse reactions are gastrointestinal, including nausea and vomiting
- Female patients found to be more susceptible than male patients to gastrointestinal adverse reactions and weight loss
Pregnancy & Lactation
- Not recommended during pregnancy unless clearly necessary
- Excreted into milk in animals
- Not known if excreted into human milk
- Women on rivastigmine should not breast-feed
Precautions & Warnings
- Patients with sick sinus syndrome or conduction defects, respiratory diseases
- Cholinergic stimulation may increase gastric acid secretion
- May exacerbate urinary obstruction and seizures
- Monitor body weight
- Asthma or obstructive pulmonary disease
- May worsen extrapyramidal symptoms
Use in Special Populations
- Dosing recommendations for renal and hepatic impairment
- Not recommended for use in children
Overdose Effects
- Accidental overdose may not be associated with clinical signs or symptoms
- Symptoms may include nausea, vomiting, diarrhoea, hypertension, hallucinations, bradycardia, or syncope
- Conservative management recommended in cases of asymptomatic overdose
- Antiemetics and symptomatic treatment may be necessary
- In massive overdose, atropine can be used
