Indications
- Treatment of chorea associated with Huntington's disease
Pharmacology
- Vesicular monoamine transporter-2 (VMAT-2) inhibitor
Dosage & Administration
- Can be taken with or without food
- Requires careful weekly titration
- Individualized dosing
- Pediatric & geriatric use not established
Interaction
- Strong CYP2D6 Inhibitors may require reduction in dose
- Reserpine should not be used concomitantly
- No interaction with food
Contraindications
- Actively suicidal or untreated/undertreated depression
- Hepatic impairment
- Concurrent use of MAOIs, reserpine, deutetrabenazine, or valbenazine
Side Effects
- Common: sedation/somnolence, fatigue, insomnia, depression, akathisia, anxiety, nausea
- Rare: Suicidal thoughts, neuroleptic malignant syndrome, pneumonia
Pregnancy & Lactation
- No adequate data on developmental risk in pregnant women
- No data on presence in human milk
Precautions & Warnings
- Maximum daily dose should not exceed 50 mg/day
- Maximum single dose should not exceed 25 mg if administered with strong CYP2D6 inhibitor
- Periodic re-evaluation of benefit and adverse effects
- Impairment of ability to drive or operate machinery
- Not recommended in combination with drugs that prolong QTc
Overdose Effects
- Signs and symptoms may include drowsiness, sweating, hypothermia
