Indications
- Treatment of chorea associated with Huntington's disease
Pharmacology
- VMAT-2 inhibitor
- Symptomatic treatment of hyperkinetic disorders
Dosage & Administration
- Can be taken with or without food
- Individualization of dose with weekly titration
- Dosing schedule for titration
- Requires genotyping for CYP2D6 metabolizers
- Max daily dose for poor and extensive metabolizers
- Safety and effectiveness not established in pediatric and geriatric patients
Interactions
- Interaction with strong CYP2D6 Inhibitors
- Interaction with Reserpine
- No interaction with food
Contraindications
- Contraindicated in actively suicidal patients or untreated/undertreated depression
- Contraindicated in hepatic impairment
- Contraindicated in concurrent use with MAOIs, reserpine, deutetrabenazine, or valbenazine
Side Effects
- Common adverse reactions: sedation/somnolence, fatigue, insomnia, depression, akathisia, anxiety, nausea
- Rare adverse reactions: suicidal thoughts, neuroleptic malignant syndrome, pneumonia
Pregnancy & Lactation
- No adequate data on developmental risk in pregnant women
- No data on presence in human milk
Precautions & Warnings
- Periodic re-evaluation of benefit and potential adverse effects
- Maximum daily dose and single dose with strong CYP2D6 inhibitor
- Discontinue treatment in case of Neuroleptic Malignant Syndrome
- Reduce or discontinue dose if experiencing certain adverse effects
- May impair ability to drive or operate machinery
- Not recommended in combination with drugs prolonging QTc
