Indications
- Primary hyperlipidemia and mixed dyslipidemia
- Hypertriglyceredemia
- Primary dysbetalipoproteinemia
- Homozygous familial hypercholesterolemia (HoFH)
- Atherosclerosis progression
- Pediatric patients with heterozygous familial hypercholesterolemia (HeFH)
- Risk reduction of Myocardial Infarction (MI), stroke, and arterial revascularization procedures
Pharmacology
Selective inhibitor of HMG-CoA reductase, increases hepatic LDL receptors and inhibits hepatic synthesis of VLDL and LDL particles
Dosage & Administration
- 5-20 mg/day for Heterozygous Familial Hypercholesterolemia in Pediatric Patients (10 to 17 years of age); max 20 mg/day
- 40 mg once daily for Very High Cholesterol and High Risk of MI or Stroke
- 20 mg once daily for Homozygous Familial Hypercholesterolemia
- Initiation with 5 mg once daily for Asian Patients
- Limit dose to 5 mg once daily with Cyclosporine
- Limit dose to 10 mg once daily with Lopinavir/Ritonavir or Atazanavir/Ritonavir
- Consider reduction in dose when used in combination with Niacin or Fenofibrate
- Avoid combination therapy with Gemfibrozil
- Start at 5 mg once daily for patients with severe renal impairment (ClCr<30 mL/min/1.73 m²)
Interaction
- Cyclosporine increases exposure
- Gemfibrosil combination to be avoided
- Lopinavir/Ritonavir or atazanavir/ritonavir combination increases exposure
- Coumarin anticoagulants prolong international normalized ratio (INR)
- Concomitant lipid-lowering therapies may increase the risk of skeletal muscle effects
Contraindications
- Known hypersensitivity to product components
- Liver disease
- Pregnant women and women who may become pregnant
- Nursing mothers

Pregnancy & Lactation
Should be administered with caution to women of childbearing age, discontinuation in case of pregnancy, excretion in human milk not confirmed
Precautions & Warnings
Caution in patients with predisposing factors for myopathy, discontinue therapy if markedly elevated creatinine kinase levels occur, liver enzyme tests recommended before and during therapy
Overdose Effects
No specific treatment, symptomatic treatment recommended, hemodialysis doesn't significantly enhance clearance
