Mode of Action
- Absorption is greater on an empty stomach
- Readily absorbed
- Time to peak concentration in adults is 2.1 to 3.2 hours for oral dosage forms
- High concentration in phagocytes
- Concentration in tissues over 50 times higher than in plasma
- Active against most isolates of specific microorganisms
- Binds to the 50S ribosomal subunit of susceptible microorganisms
- Interferes with microbial protein synthesis
Dosage
- 500 mg once daily orally for 3 days for adults
- 1 gm given as a single dose for sexually transmitted diseases in adults
- Dosage varies for children based on body weight
- Recommended dose for injection for treatment of adult patients with community-acquired pneumonia and pelvic inflammatory disease
- Safety and effectiveness not established for children or adolescents under 16 years

Interaction
- Interaction with antacids, carbamazepine, cyclosporine, digoxin, ergot derivatives, methylprednisolone, theophylline, warfarin, terfenadine
Contraindications
- Hypersensitivity to Azithromycin or any other macrolide antibiotic
- Co-administration of ergot derivatives
- Patients with hepatic diseases
Side Effects
- Mild to moderate gastrointestinal side-effects
- Allergic reactions such as rash or photosensitivity
- Rare reports of serious hypersensitivity reactions
- Reversible elevations in liver transaminases
- Rare cases of cholestatic jaundice
- Transient mild reductions in neutrophil counts
- Reversible hearing impairment in some patients with higher doses for prolonged periods
Pregnancy & Lactation
- Pregnancy Category B
- No evidence of harm to the fetus in animal reproduction studies
- Caution during pregnancy and breastfeeding due to lack of adequate and well controlled studies
Precautions & Warnings
- Rare serious allergic reactions reported
- Exercise caution in patients with renal impairment
- Avoid use in patients with severe hepatic impairment
Overdose Effects
- Typical symptoms include hearing loss, severe nausea, vomiting, and diarrhea
- Gastric lavage and general supportive measures indicated in case of overdose
